Anterior cruciate ligament reconstruction (ACL-R) is probably the most performed arthroscopic knee surgical procedure. The technique of ACL-R continues to evolve, and the search for the Holy Trial continues. We have not yet decided the ideal “Gold Standard” graft source for ACL-R. Newer fixation devices have made the use of shorter grafts possible. The quest for the ideal femoral and tibial tunnel position yet seems an ongoing enigma. Several evolving technologies have been investigated to address this issue. Over the past decade, the role of secondary stabilizing structures in an ACL injured has been investigated in detail. The indications for the use of additional procedures for treating these secondary injuries are now becoming clearer. We are gradually inching closer to improved outcomes and ACL-R survival rates.

      Short graft anterior cruciate igament reconstruction is increasing in popularity for performing a primary ACLR. The short graft coupled with the all-inside technique using closed sockets and suspensory fixation at both femoral and tibial ends are its defining features. The outcomes of this technique have been comparable to well established transportal ACLR techniques. It has the benefits of preserved hamstring strength and less pain attributed to transtibial drilling. However, there is a learning curve involved and will require time before mastery of the technique.
